A job hazard analysis template helps a team examine work before exposure begins. It breaks the job into meaningful steps, identifies how people, equipment, materials and the environment could interact, and records the approved controls, owners and conditions needed for the work to proceed.

This generic template is not a permit, method statement, certification or professional safety advice. Applicable law, site rules, approved risk methods, equipment instructions and qualified safety or engineering judgment take priority. Workers must retain stop-work authority under the organization’s procedure when conditions are unsafe or unclear.

Define the job, boundaries and work team

Describe the specific task, location, equipment, materials, planned date, shift and expected duration. Define where the job starts and ends. “Maintain machine” is too broad; identify preparation, isolation, access, disassembly, inspection, reassembly, testing and return to service as applicable.

Name the work owner, competent supervisor, participating workers, contractors and specialists. Record required competence, licenses or authorizations without copying sensitive personnel records into the JHA. Include people who perform the work; practical knowledge often reveals exposures that are absent from desk-based planning.

List interfaces with operations, the public, adjacent teams and simultaneous work. Clarify who can authorize start, suspend work, approve changes and return equipment or an area to service.

Define the planned work state precisely. Production equipment may be stopped, isolated, energized for diagnosis or operated in a special mode, and each state changes the exposure. Record expected staffing and supervision. If staffing falls below the approved plan, pause and obtain direction rather than redistributing critical duties informally.

Gather requirements and observe the work

Review approved procedures, manuals, safety data, prior JHAs, permits, incident learning and change records. Walk the area with experienced workers when feasible. Observe normal work and credible non-routine conditions without exposing anyone merely to complete the analysis.

Confirm energy sources, access, environmental conditions, substances, tools, lifting needs, ergonomics, communication and emergency arrangements. Check whether the task changes across shifts or modes. Treat missing information as an action or hold point, not an invitation to guess.

Use the organization’s approved risk definitions and control hierarchy. Do not invent scores or acceptance thresholds. A signature should show participation or authorization as defined by policy, not waive rights or transfer management responsibility.

Break the job into observable steps

Write steps in the order work occurs, using action verbs and enough detail to expose meaningful changes in risk. Avoid both extremes: one vague step hides hazards, while every hand movement makes the analysis unusable. A step should usually represent a change in activity, position, energy state, tool or interface.

Include setup, access, material handling, temporary states, testing, cleanup and restoration. Many incidents arise during transitions rather than the primary production step. Identify who performs each step and where other people could enter the exposure area.

Include reasonably foreseeable recovery actions such as clearing a jam, retrieving a dropped item or responding to a failed test, but do not normalize improvised fault recovery. Where the approved response is to stop, isolate and call a specialist, state that boundary clearly.

Validate the sequence with workers. If the real method differs from the approved procedure, stop the planning assumption and route the discrepancy through the authorized change process instead of normalizing an undocumented workaround.

Identify hazards and credible exposure paths

For each step, ask what energy, motion, substance, condition or human-factor demand could cause harm. Consider mechanical, electrical, pressure, thermal, chemical, biological, gravity, noise, vibration, radiation, ergonomic, environmental and psychosocial factors as relevant to the work.

Describe the exposure path rather than listing a category alone. “Stored pressure released when fitting is loosened” is more actionable than “pressure hazard.” Identify workers, contractors, visitors, the public, equipment or environment that could be affected.

Use prior events and near misses as prompts, not predictions. Do not invent likelihood or severity. Apply the approved risk register template or site risk method when broader tracking is needed, and escalate uncertain specialist hazards for qualified assessment.

Select controls using the approved hierarchy

Start with whether the hazard can be eliminated or the task redesigned. Then consider substitution, engineered safeguards, administrative controls and personal protective equipment according to the organization’s approved hierarchy. PPE can be necessary, but it should not silently replace feasible higher-order controls.

Write controls so they can be verified: identify the isolation point and verifier, guard type, exclusion boundary, ventilation requirement, lifting device, approved procedure or communication method. “Be careful” and “use proper PPE” are not sufficient descriptions.

Assign each additional control to an owner and define completion evidence. Qualified owners approve residual risk and determine whether work can proceed. The JHA must not authorize deviations from manufacturer instructions or mandatory safeguards.

Check whether controls create new hazards or conflict with one another. Gloves can affect dexterity, barriers can alter access, ventilation can change pressure relationships and communication equipment can be unreliable in noise. Resolve those interactions with competent owners and test controls under safe, approved conditions before relying on them.

Connect permits, isolation and emergency arrangements

List required permits, isolations, lockout or tagout steps, atmospheric testing, access controls, spotters and specialist authorizations. Reference their controlled records rather than duplicating details that may change. A JHA marked complete does not mean those separate controls are active.

Define hold points before hazardous transitions, such as opening equipment, entering a restricted space, lifting, energizing for test or returning to service. State who verifies conditions and who releases each hold point.

Confirm emergency communication, rescue or first-aid arrangements required by the site and task. Do not assign rescue duties or specify technical rescue methods without competent planning, equipment and authorization.

Where multiple permits apply, identify their coordination owner and common validity window. A permit issued for one activity should not be assumed to cover adjacent work, a changed crew or a later shift. Confirm cancellation, suspension and handback requirements before work starts.

Brief the team and invite challenge

Conduct the pre-start briefing where participants can review the sequence, hazards, controls, interfaces and stop criteria. Use accessible language and interpretation where needed. Ask workers to explain critical controls in their own words rather than relying only on signatures.

Create a clear route for questions, disagreement and new information. Power differences can suppress challenge, so supervisors should explicitly invite pause requests without retaliation. Record material questions and changes, but avoid collecting unnecessary health or personal information.

Verify controls and manage change during work

Before start, a competent person should confirm that site conditions match the analysis and that controls are installed, functional and understood. During work, supervise in proportion to risk and competence. Evidence may include permit status, isolation verification, equipment checks or a documented hold-point release under approved procedures.

Pause and reassess when scope, sequence, crew, equipment, materials, weather, occupancy or adjacent work changes, or when a control fails. Do not edit the record after the fact to make an unassessed change appear planned. Preserve the revision and communicate it to everyone affected.

If an incident or near miss occurs, follow the emergency and reporting process first. Use a corrective action report template later to investigate and track improvements without assigning blame prematurely.
