A fire door inspection checklist helps a competent person record the identity, visible condition and operation of each door assembly, then route defects to qualified owners. Its value comes from specific evidence and controlled follow-through, not from a row of unchecked assumptions.
This checklist is not certification, authorization or proof of fire performance, safety or compliance. Qualified owners must apply applicable law, approved fire strategy, manufacturer instructions, certification details, site-specific controls, organizational policy and professional judgment. Never dismantle, adjust or test beyond competence and authorization.
Establish scope, competence and inspection basis
Define which buildings, floors, compartments and door types are included. Identify fire doors, smoke-control doors, doors on escape routes, service riser doors, shutters or specialist assemblies according to the approved building records. A door’s appearance alone may not establish its intended function.
Assign a responsible person, competent inspector, facilities owner and repair authority. Clarify what the visual and operational inspection covers, when a specialist is required, and who can accept temporary controls or return an assembly to service.
Set frequency using the applicable requirements, fire risk assessment, approved strategy, use, condition and local change. Higher traffic, repeated damage or vulnerable locations may need different oversight, but the qualified owner must determine the program.
Confirm door identity and controlled records
Give every assembly a stable asset ID and precise location. Record building, level, room or corridor, door number, orientation and whether it is a single or paired set. Photographs may assist identification if authorized and securely handled.
Link approved drawings, schedules, prior inspections, maintenance history and certification information where applicable. Do not copy a label from another door or infer a rating from paint, thickness or location. If identity or intended performance is uncertain, record that uncertainty and escalate it.

Prepare the inspection safely
Coordinate access, occupants and local operations. Follow site procedures for work areas, ladders, restricted rooms, security doors and automatic systems. Do not obstruct an escape route, isolate an alarm interface or disable access control without authorization.
Use only approved, suitable measuring tools and the defined inspection method. Keep hands clear of closing edges and hinges. Do not force a door, remove hardware, lift seals or carry out destructive checks merely to complete a field.
Observe the assembly from both sides where safely accessible. Record inaccessible areas and limitations. A partial inspection should remain partial rather than being marked satisfactory by assumption.

Adapt the fields to the specific assembly and approved inspection regime. A generic gap value, hardware rule or rating should never override the authoritative design and applicable requirements.
Inspect leaf, frame and surrounding interface
Check the leaf faces and edges for holes, splits, delamination, warping, impact damage, missing material, unapproved penetrations or signs of alteration. Examine the frame and visible fixings for damage, movement, corrosion or gaps that require qualified assessment.
Look at the interface with the surrounding wall where visible. Cracking, unsealed work or later services near the opening may need specialist review. Do not probe, remove finishes or assume hidden construction from surface appearance.
For paired doors, inspect both leaves, meeting edges and the interaction between them. Record exact locations rather than “door damaged.” The quality inspection report template provides a useful evidence-and-nonconformity structure, but acceptance criteria remain fire-door specific.
Check gaps, seals, glazing and identification
Measure clearances only with the approved method and compare them with the correct requirement for that assembly. Record actual observations and locations. Do not apply one generic limit to every fire door, threshold, smoke-control function or specialist design.
Inspect intumescent and smoke seals where applicable for continuity, secure location, damage, contamination, paint or missing sections. Do not pull or trim them during inspection. A qualified person should determine replacement materials and compatibility.
Check glazing, beads, panels, letter plates, viewers and other apertures for visible damage, looseness or unapproved change. Record labels, plugs or identification without obscuring or removing them. Missing identification creates an evidence gap; it does not by itself prove either performance or failure.
Test closing, latching and coordinated operation
Use the approved operational test from safe positions. Observe whether the door moves freely, closes fully and engages the latch where the design requires it. Test from required opening positions according to the inspection basis; do not invent a sequence.
For paired leaves, check the coordinator and closing sequence where applicable. Assess hold-open or free-swing arrangements only within the approved fire-alarm and maintenance procedure. Never trigger, isolate or bypass a system without authorization.
Floor coverings, air pressure, warped components and poorly positioned items can affect closing. Record the condition present during the test. Do not compensate by slamming the door or adjusting a closer without qualified approval.
Examine hinges, hardware and access-control interfaces
Inspect hinges or pivots, closers, latches, locks, handles, panic or emergency hardware, coordinators, guards and protective plates applicable to the assembly. Look for missing or loose components, visible damage, oil leakage, binding and unauthorized substitutions.
Access-control equipment, cables, magnets and transfer devices can affect integrity and operation. Confirm that additions are documented and assessed through the responsible process. Security convenience must not silently defeat the approved fire-door function, and fire considerations do not authorize unsafe security changes.

Check obstructions and user-created conditions
Look for wedges, hooks, furniture, floor mats, stored items, decorations, cables or damaged flooring that obstruct movement or prevent closure. Record the direct condition and follow the site’s immediate escalation process. Do not create a trip or manual-handling hazard while moving items outside your authority.
Check that signage and user instructions required by the approved strategy are present and legible. Avoid adding unofficial signs that conflict with accessibility, security or building standards. Repeated wedging may indicate an operational need that requires an approved solution rather than repeated removal alone.

Record defects, interim controls and repairs
Describe the component, exact location, observed condition, functional result, evidence and inspection limitation. Avoid unsupported statements about fire resistance or cause. Assign severity only through the organization’s approved criteria and competent review.
Escalate promptly. The responsible owner decides interim controls, access restrictions, patrols, repair priority or other measures under the approved fire strategy and risk process. A checklist is not permission to leave a defective door in service or to invent a temporary repair.
Create a controlled work order and require compatible parts, qualified work and configuration records. Use a repair handover checklist to transfer the defect clearly. After repair, reinspect affected features and retain before-and-after evidence plus closure authority.
Handle evidence, privacy and assisted capture
Photographs should establish door identity, component location and scale where useful. Avoid capturing occupants, access codes, security layouts or personal information unnecessarily. Store evidence under approved access and retention controls.

Quality-check records and program performance
Before closing an inspection, reconcile every scheduled door to a result, limitation or exception. Check asset IDs, evidence links, measurement units, defect ownership and repair references. Sample “satisfactory” records to ensure fields reflect actual checks rather than repeated boilerplate.
