A field sampling log template preserves the connection between a physical sample and the conditions in which it was collected. That connection supports qualified review later. The log does not create a sampling plan, authorize collection, validate a method or interpret a laboratory result.
Use this framework only within an approved program. Trained and authorized personnel must follow the sampling plan, safety controls, method, preservation requirements, custody procedure and laboratory instructions that apply to the project and jurisdiction.
Start from the approved sampling plan
Record the controlled plan, project and method references before entering the field. The log should show why each sample exists and how it maps to an approved location and objective.

Do not improvise new locations, analytes or methods because a blank row remains. Route proposed changes to the person authorized by the plan and record the decision separately.
Design sample IDs that survive handoffs
A sample ID should be unique, legible and stable from container to field log, custody form and laboratory system. Avoid identifiers that depend only on a collector’s memory or an easily confused sequence.

Apply labels before collection when the method permits, then verify container and log together. Never reuse an abandoned ID without documenting its status.
Record location so it can be reproduced
Use the location system specified by the plan: station, grid, room, asset, depth, coordinates or another stable reference. Record the coordinate system, units and device where relevant. A screenshot or map pin without its coordinate basis may be unusable later.

Capture conditions at the time of collection
Record date, time, weather, operating state and observable site conditions required by the plan. Separate direct observations from interpretations. If conditions change between samples, create a new timestamped entry rather than applying one description to the entire day.

Do not identify an unknown material by appearance. Describe it factually and route identification to the authorized method and qualified reviewer.
Document equipment, preparation and quality control
List equipment IDs, checks, cleaning or decontamination references and calibration or verification status where the controlled method requires them. Record blanks, duplicates and other quality-control samples with the same care as primary samples.
Quality-control records should remain linked to the samples they inform. Do not silently exclude a failed check or repeat collection. Preserve the event, response and reviewer decision.

Record collection and preservation exactly
Capture the method reference, container type, volume or quantity where required, preservation action, temperature-control status and holding-time basis. Use exact controlled terminology rather than paraphrasing technical requirements from memory.

Never invent a value to complete the table. Mark missing or inapplicable fields visibly and follow the local discrepancy process.
Handle deviations without rewriting history
A deviation can include a changed location, damaged container, missed preservation step, equipment issue, access restriction or identification discrepancy. Record the facts at the time, notify the appropriate role and preserve the original entry.

Do not decide on your own that a deviation is immaterial. The qualified reviewer and laboratory must assess impact under the approved program.
Maintain chain of custody at every transfer
Custody records show possession, security and transfer. Use one unambiguous line for each handoff: released by, received by, date, time, sample or cooler IDs, condition and purpose. Both people should follow the approved acknowledgment process.

If a seal is broken, a cooler is delayed or custody information conflicts, preserve the evidence and follow the laboratory and project procedure. Never guess the missing transfer.
Reconcile laboratory receipt
At receipt, link the laboratory acknowledgment, condition notes, temperatures where required, discrepancies and assigned laboratory IDs back to field sample IDs. Resolve mismatches through a documented authorized process.
The field team should not alter the original log to match a later laboratory entry. Add a traceable correction or reconciliation note that preserves both states and the reviewer’s decision.
Protect sensitive site and personal information
Sampling records may contain precise locations, infrastructure details, landholder data or proprietary operations. Collect only what the plan requires, use approved systems and restrict access and retention.
Photographs and recorded conversations require separate authorization. Keep unrelated people, screens, badges and security features out of frame. Apply the governance principles in the meeting recording consent form when an agreed debrief will be captured.

Review completeness before the team disperses
Reconcile planned samples, collected samples, unused IDs, quality-control samples, custody forms and laboratory requests while the context is still available. Check that locations are reproducible and that every correction preserves the original entry.

Preserve corrections, versions and long-term traceability
Field records are often completed under time pressure, so correction rules matter. Never erase an original entry or overwrite a file without a trace. Use the approved correction method to show the original value, revised value, reason, author and timestamp. A later reader should be able to understand both the field state and the subsequent review.
Version the blank template separately from completed logs. Changing a prompt for future work must not alter the meaning of historical records. Record which template and sampling-plan versions were used for each event. When a method or naming convention changes, provide a controlled mapping rather than silently normalizing old sample IDs.
Reconciliation should cover more than total counts. Compare each planned location with collection status, each container with its sample ID, each custody line with cooler contents and each laboratory receipt with the request. Investigate duplicates, gaps and transposed characters. A set can have the expected number of containers and still contain the wrong identity mapping.
Retain enough context for qualified interpretation without turning the field log into a laboratory report. Weather, process state, access limits and deviations belong in the field record. Analytical calculations, validation and conclusions belong in their controlled review record. Link the two through stable IDs so neither document has to imitate the other.
At project close, apply the approved retention, access and disposal schedule. Do not keep convenience copies indefinitely on personal devices or in chat. Confirm that photographs, maps and debrief notes follow the same governed evidence trail. If a record is subject to a hold or investigation, authorized instructions override routine disposal. These controls protect sample integrity and people whose property, work or location information appears in the record.
Before archiving, create a completion note that identifies the authoritative record set, unresolved discrepancies and responsible custodian. Confirm that every referenced attachment exists and that access will not depend on one team member’s account. If a paper original is required, document its storage and any approved digitization. Long-term traceability is strongest when the index remains simple, stable and understandable without private background knowledge.
